HOURS AND APPOINTMENTS
Our office hours by appointment.  Our office hours are 9:00 a.m. until 4:45 p.m., Monday through Friday.  Telephone hours are 9:00 a.m. through 4:40 p.m.

If you are unable to keep an appointment, please call as soon as possible so your appointment can be rescheduled and your time may be used for another patient.  The time allotted for each appointment depends on the nature of your visit, so please give the receptionist enough information so she may assign the proper time and priority to your needs.  

MEDICAL RECORDS
All of your medical records are confidential, and no information is given over the telephone without your written consent.  Your written authorization is required before your records will be released to insurance companies or others.  There is a minimum $10.00 processing fee for reproduction and release of records.

Our phone is open 9 to 4:45 including lunch hours.

Call 432-1413 on weekend or after office hours to find out the contact number, only if you have emergency problem.

If you are calling for a prescription refill, please call during the telephone hours so we can check your records to make sure a refill is the best course of treatment.  You also should have the name and telephone number of your pharmacy available before your call.

FEES AND INSURANCE
We expect payment at the time of service either in full or as designated by your insurance plan.  As a  convenience for you, we accept cash, check, MasterCard, Visa or Debit Card as payment options.

ANSWERING SERVICE / EMERGENCIES
Emergency calls are accepted at all hours.  After hours, the doctor on call may be reached by calling (559) 432-1413.  If you have what you consider to be a true emergency, have the answering service contact the doctor immediately, and please stay off your telephone until the doctor returns your call.
